Your electronic health record (EHR) is an electronic version of your health information that is maintained by your doctors. Your health record may include a collection of your past doctor visits, treatments received, test results and medications. Accurate, real-time health records are a vital part of receiving the best care possible. Right now, your health record information may be spread across multiple doctors and hospitals that you have visited in the past. For your health care providers to make the most effective decisions on your care, your providers need to be able to see information about you such as your conditions, test results, treatments and the medications you may be taking.
Health information exchange (HIE) is an electronic, patient-centered approach that assists with the transfer and sharing of health related information between physicians, hospitals, and other health care providers at the time it is needed to help them make clinical decisions. An HIE provides authorized care providers and physicians with quick, reliable, and secure access to patient health records from different EHR systems.

Maintaining your information privacy is a priority of the Tiger Institute Health Alliance. Only health care providers who are Members of the Tiger Institute Health Alliance and have a relationship with you as their patient will have access to your health information on the HIE to assist them with providing you with the most appropriate care.
All Tiger Institute Health Alliance Member organizations and their health care providers must abide by State and Federal privacy laws, the Health Insurance Portability and Accountability Act (HIPAA) of 1996, the Privacy Act of 1974, and the security provisions for health information in the American Recovery and Reinvestment Act (ARRA). Additionally, all Tiger Institute Health Alliance Members use state-of-the-art information technology to keep health information safe and secure. Only participating health care providers who are authorized by HIPPA laws to view your health records will have privileges to access the Tiger Institute Health Alliance HIE. Routine audits are also performed to ensure compliance to applicable laws and user agreements.
If you are a patient at a Tiger Institute Health Alliance Member organization, you will be notified that they are participating in health information exchange. Your information will automatically be shared so if you wish to participate there is no action required on your part. If you do not want to participate, ask your provider about completing a form to opt-out of the health information exchange. Please understand that if you opt-out of the health information exchange you will limit your healthcare providers’ ability to provide the best care for you. By opting out of the health information exchange you accept the risks associated with that decision. Your health information will continue to be shared (without your specific authorization) among providers and health plans through other means, such as mail, fax or secure e-mail. In other words, opting out of the health information exchange will not prevent your providers from seeing your complete medical record.
If you decide that you would no longer like to have your health information accessible to Tiger Institute Health Alliance Members, you may sign an “Opt-Out” form provided by a Tiger Institute Health Alliance Member organization. Upon canceling your participation, all information already in the system will be blocked from access.
