Will be responsible for providing the day to day management of a busy expanding medical clinic, giving direction, strategic planning, organizational goals, process improvement & procedures, budgets, evaluation of nursing care, quality improvement, risk management, hiring and supervision of nursing staff and reporting for CHCCMO. Interprets and supports the purposes, philosophy and objectives of the agency. Performs a variety of duties to provide and coordinate quality nursing care to patients. Participates with the Chief Clinical Officer in establishing specific goals, determines work procedures and expedites work flow, insures compliance with all operating policies and procedures and Missouri Department of Health regulations and Federal Health Care Administration regulations, Investigates reported and suspected HIPAA concerns ,communicates with and supervises personnel, provides daily reports and other periodic reports as required.

Performs all the duties of an RN, LPN or certified Medical Assistant when required.

Ideal candidate for this position would have 2-3 years previous management experience in a medical clinic setting

Must be a licensed NP with 3-5 years experience working in a Pediatric setting required for this position. Community Health Center of Central Missouri is seeking experienced Pediatric LPNs/RNs  to join our ever-growing family. Full-time opportunities available to work in Community Health setting with compassionate/caring providers, in Pediatrics. Ideal candidate would have a minimum of 2 years experience working in a healthcare setting with Pediatric patients.

(no weekend, no major holidays, and no on-call for these positions).

Performs a variety of duties to provide and coordinate medical care to CHCCMO pediatric patients.  Participates with the Medical Director in establishing clinical protocols, participating in peer review, and monitoring and assuring quality.  Specific goals;  insures compliance with all operating policies and procedures and Missouri Department of Health regulations and Federal Health Care Administration regulations; communicates with and supervises  personnel; provides daily reports and other periodic reports as required. Performs all the duties of medical provider when required and is expected to respond to medical emergencies that take place at CHCCMO sites and other locations when working for or representing CHCCMO.

Seeking a full-time experienced motivated Phlebotomist who is able to work independently to perform blood draws and some minor lab testing. This position would be Monday – Friday, a day position with no weekends, evenings or nights.

Provides the delivery and documentation of age/population specific psychotherapy and social services. Completes psychosocial assessment appropriate to patient type and reason for referral to assist team in development of care plan. Promotes safe and therapeutic patient care through consultation and supervision, and effective communication.

THE IDEAL CANDIDATE WILL:

  • Proactively identify and evaluate patients and families for case management from a variety of sources such as through discharge/disposition planning, referrals, the healthcare system, employers and facility staff.
  • Develop an appropriate patient-specific plan of care to include short and long term goals, objectives and actions.
  • Guide the patient and caregiver through the healthcare system, maximizing use of resources, and executes plan of care.
  • Ensure coordination of care delivery process, to include alternate healthcare settings and the home environment, for the purposes of enhancing the patient’s health and wellness.
  • Document and update the treatment plan as needed in accordance with existing guidelines.
  • Counsel patients and interpret to professional and lay groups the special psychosocial needs of patients by identifying psychosocial disorders.
  • Perform short term counseling and crisis intervention as necessary.
  • Assist in the implementation of community and social programs as a professional counselor in accordance with CHCCMO grants/contracts with federal, state, and local agencies and mandates of service requirements with those grants/contracts.
  • Coordinate with clinical, health education, community health and other staff members in the provision of services to patients. Provide referrals for services needed
  • Conduct health related educational presentations to both adults and teens in school and clinical settings.
  • A current valid Missouri license having no adverse action pending from any state or national license/certification agency. Social worker license is defined as a license that allows the Social Worker to work independently without clinical supervision.
  • A minimum of 2 years full-time experience in professional case management or discharge planning programs for adults, children, families, seniors, and groups.

Performs a variety of duties to provide and coordinate behavioral health care to CHCCMO patients and oversight of the Behavioral Health Department.  Participates with the Medical Director in establishing clinical protocols, participating in peer review, and monitoring and assuring quality.  Specific goals;  insures compliance with all operating policies and procedures and Missouri Department of Mental Health regulations and Federal Health Care Administration regulations; communicates with and supervises  personnel; provides daily reports and other periodic reports as required. Performs all the duties of a psychologist when required and is expected to respond to behavioral health crises/emergencies that take place at CHCCM sites and other locations when working for or representing CHCCM.
