Chronic Disease Management
Almost half of all Americans live with a chronic medical condition and almost half of those people have multiple conditions. As a result, Community Health Center of Central Missouri has established a Chronic Care program to help our patients manage diabetes, asthma, depression, pain and other chronic conditions.
